In the table below, you will need to enter either your six-digit or nine-digit Fisher Customer Account Number. Entering a

Fisher Customer account number will enable you to receive all invoices for the account, no matter what department (nine-digit account) ordered the product. Entering a
nine-digit
Fisher Customer account number will enable you to receive only those invoices that are billing the product shipped to that
nine-digit
department account.
